The clearance of uric acid, of total ketones, and of creatinine was determined during
a 10-day fast in 4 obese women and in response to an acute infusion of β-hydroxybutyrate.
A brisk fall followed by a plateau of uric acid clearance was concomitant with a steady
rise of plasma ketones; thereafter the changes were not uniform. This inverse relationship
in the early stages of fasting was duplicated with exogenous ketosis. The inhibition
of renal excretion of uric acid by the ketosis of fasting is confirmed and some mechanisms
for this effect are suggested.
